Mattress and Back Pain

How your mattress affects spinal health, signs it's causing pain, and what types of support help most.

Quick Answer

Your mattress directly affects spinal alignment during sleep. A mattress that's too soft lets the hips sink and curves the spine downward. A mattress that's too firm pushes against the body and prevents natural curvature. Both cause muscle strain and pain. Medium-firm is research-backed as the best overall option for back health.

How Mattresses Affect the Spine

Your spine has natural curves — cervical (neck), thoracic (mid-back), and lumbar (lower back). During sleep, the mattress must support these curves while allowing pressure points (shoulders, hips) to sink in appropriately.

A properly supportive mattress keeps the spine in neutral alignment — the same posture you'd have standing with good form. When alignment is off, muscles compensate overnight, leading to morning stiffness and pain.

Different sleep positions require different support: side sleepers need shoulder/hip contouring, back sleepers need lumbar fill, and stomach sleepers need firm hip support.

Signs Your Mattress May Be Causing Pain

  • Morning stiffness that fades by mid-morning — the classic sign of mattress-related back pain
  • Pain that's worse after sleeping, better after moving
  • Visible sagging or body impressions in the mattress
  • The mattress is 7+ years old
  • You sleep better in hotels or on other beds
  • Tossing and turning significantly increased

What Types of Support Help Most

Zoned support — Firmer under the lumbar area, softer at the shoulders. This maintains the natural lumbar curve while allowing shoulder contouring. Found in Stearns & Foster IntelliCoil and Helix zoned coils.

Responsive materials — Pocketed coils and latex push back proportionally to body weight, providing more support where you're heaviest (hips) and less where you're lightest (shoulders).

Adjustable bases — Elevating the legs takes pressure off the lower back. The "zero gravity" position decompresses the spine. Adjustable bed FAQ →

Proper firmness — Medium-firm (6-7) for most people. Firmness guide for back pain →

When to Replace the Mattress

If your mattress is causing back pain, no amount of toppers or adjustments will fully fix a structurally compromised mattress. Replace if:

  • Sagging exceeds 1-1.5 inches
  • The mattress is over 7-8 years old
  • You've tried firmness adjustments (topper, different pillow) without improvement
  • The mattress no longer bounces back when you get up
